This site gives you information about how to opt out of care.data. It provides a simple letter you can download and post to your GP.

Care.data is an initative of the Health and Social Care Information Centre (HSCIC) http://www.hscic.gov.uk/, a new body with the role of “collecting, analysing and presenting national health and social care data” with the aim of planning and monitoring services. This data will be shared with other public and private organisations. However, the key issue has been the lack of processes for ensuring the confidentiality of the data.
